چکیده

Abstract Background Little is known about the outcomes of liver cirrhosis patients with severe aortic stenosis (AS) who undergo transcatheter valve replacement (TAVR). Methods We undertook a retrospective analysis consecutive symptomatic AS underwent transfemoral-TAVR Sapien-3 at our Clinic between April 2015 and December 2018, yielding 32 on imaging including ultrasound and/or computed tomography. Their baseline characteristics, procedural long-term after TAVR non-cirrhotic group were compared, along their management strategies as per hepatology team. Results Among 1028 patients, assigned to cirrhosis, 996 non-cirrhosis (control) group. Compared control cirrhotic slightly younger in age (74.5 vs 81.2 years), had higher BMI (31.3 29.3), incidence prior history myocardial infarction (38% 33%). Baseline variables smoking, hypertension, diabetes, atrial fibrillation comparable both groups. (n=32), most common etiologies non-alcoholic steatohepatitis (NASH) (37.5%), Alcoholism (18.75%), Hepatitis C (12.5%). The mean MELD-NA score was 11.8 67% Child PUGH Class A 33% B all ≥5. 53% (n=17) evaluated by Hepatology 12.5% (n=4) for transplant but only 1 patient post-TAVR. similar 1-year mortality (12% 12%, p=1), lower rate 30-day new pacemaker post tavr (6% 9% p=0.85), readmission heart failure 5% p=0.12) major adverse cardiac cerebrovascular event (MACCE) (15% 14% p=0.98) Conclusion Patients undergoing concomitant demonstrate compared non- counterparts. NASH followed alcoholic found be etiology. Funding Acknowledgement Type funding sources: None. Figure 1Figure 2

Outcomes after transfemoral transcatheter aortic valve replacement.

Fro Pre kE #H & W kkC on en Va rec Ex an 2 t OBJECTIVES This study sought to determine whether outcomes for transfemoral (TF) transcatheter aortic valve replacement (TAVR) differ between the randomized controlled trial (RCT) and the subsequent NRCA (Nonrandomized Continued Access) registry of the PARTNER (Placement of AoRTic TraNscathetER Valves) trial. BACKGROUND The PARTNER RCT demonstrated t...

Transcatheter Aortic Valve Replacement: A Review Article

Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...

Transcatheter aortic valve replacement with the SAPIEN 3: a new balloon-expandable transcatheter heart valve.

OBJECTIVES The aim of this study was to demonstrate the first-in-human feasibility and short-term clinical outcomes with a new balloon-expandable transcatheter heart valve (THV). BACKGROUND The SAPIEN 3 (S3) THV incorporates a paravalvular sealing system, an active 3-dimensional coaxial positioning catheter, and is compatible with a 14-F expandable sheath. METHODS The S3 THV was implanted i...

Successful Transfemoral Edwards Sapien Aortic Valve Implantation in a Patient with Previous Mitral Valve Replacement

Transcatheter aortic valve implantation (TAVI) is a new method for patients with severe aortic stenosis at high surgical risk, such as previous cardiac surgery. The presence of mechanical mitral prosthesis might complicate TAVI because of possible interference between the both prosthesis. Some reports have already demonstrated the feasibility of TAVI in such patients. We report our experience w...
